Membership Plan Eligibility

To ensure we provide the highest level of care, patients must complete a hygiene evaluation before enrolling in any membership plan.

New Patients
New patients must complete their initial hygiene evaluation before becoming eligible for enrollment.

Full Mouth Debridement (FMD)
Patients who require a Full Mouth Debridement (FMD) before evaluation, must complete this treatment prior to enrollment. Once the FMD has been completed, enrollment may begin when the patient returns for a comprehensive evaluation. At that time, a determination will be made regarding the most appropriate plan for the patient (Adult Complete or Perio Complete).

Eligibility Exclusion

Membership plans are not available to patients receiving full arch implant treatment, including full-arch zirconia bridges, hybrid dentures, or implant-retained dentures.
